Job Description:

Under the supervision of the Director of Counseling and Psychological Services, this position provides a full range of consultation and assistance on behalf of St Cloud State University students who need access to medical, mental health, psychiatric or related care in the St Cloud community and beyond. The case manager provides coordination of care for students at risk and assists with implementation of University policies, procedures and practices regarding students at risk. The goal of this position is to promote continuity of care, assist with overcoming barriers, make referrals to support services and support the overall well-being of students. This position collaborates on initiatives related to basic needs and the food pantry. The case manager develops a network of on and off campus resources by meeting regularly with stakeholders. The case manager offers training and educational presentations on mental health and wellness related topics. This position is part of a multi-disciplinary integrated team.

For licensed mental health professionals only, the Case Manager may serve as a back-up clinician in the clinic including engaging in triage, managing crises and assessing student mental, emotional and behavioral health status and needs. In addition, in the clinical capacity, the Case Manger may do billable healthcare visits.

Provide support services for students in need or at risk.
  • Participate in the Behavioral Intervention Team (BIT) as needed.
  • Triage referrals, assist with connecting students to services, develop plans for continued care and monitor progress of student referrals through various levels of care.
  • Facilitate Emergency Room mental health assessments and follow up discharge planning to increase success for students returning to campus after a mental health crisis.
  • Interact and communicate with internal and external entities on behalf of helping a student.
  • Assist in the coordination of students' re-entry into the University due to a withdrawal due to mental health/medical reasons, if appropriate.

Provide basic needs assistance and support for students related to mental health, housing, food support, childcare, transportation and other targeted student support services.
  • Serve as point of contact for the University basic needs website.
  • Assist with social work related initiatives on campus.
  • Connect students to external and internal resources.
  • Maintain collaborative relationships with community partners.
  • Be flexible to the needs of the student population including possible co-location with campus partners.

Provide high quality case management services to students.
  • Perform case management services that address barriers to academic and personal success.
  • Advocate on behalf of students in navigating the University to maximize their success.
  • Assist students in applying for benefits, insurance, and other services/programs to ensure optimal care.
  • Facilitate and coordinate evaluation referrals (e.g. ADD/ADHD, learning disabilities, chemical dependency, eating disorders, psychiatry, and physical disabilities).
  • Assist with student academic appeals, registration petitions, reduced course-load requests and other types of leave related to mental health/medical reasons.
  • Collaborate with the St Cloud Technical and Community College on cases that overlap between agencies and as part of the Minn-State system partnership.
  • Maintain detailed client records that follow the rules and regulations of a healthcare agency including documenting case management notes in the CAPS electronic health record.
  • Collect data and produce regular reports to assist with identifying trends with case management and ways to promote student success.
  • Work professionally and effectively within a multi-disciplinary team.

Offer trainings and education to staff, faculty, students, families and the community on health and wellness related topics as well as resource education.
  • Serve as a resource to provide training and/or instruction to the campus on mental health related topics.
  • Provide education to various departments and student organizations on mental health related topics as needed.
  • Assist faculty and staff in learning about general mental health related topics as well as about internal and external social service resources.
  • Serve as part of the integrated counseling and medical team at SCSU.

Perform other duties as assigned to ensure the smooth functioning of the department and maintain the reputation of the organization as a viable business partner.

For licensed mental health professionals only, this also includes: Provide clinical/counseling care as a back-up provider at the integrated healthcare clinic including abiding by all applicable healthcare and licensing laws and regulations.
  • Provide therapy (individual and group) using a brief therapy model including crisis/triage services.
  • Maintain current licensure and remain up to date on required professional development.

Required Qualifications:

  • Bachelor's Degree in Social work or closely related field.
  • Knowledge of the modern principles, practices and techniques of the social work field.
  • Knowledge of community resources.
  • Demonstrate ability to effectively perform work of a highly sensitive and confidential nature that requires access to private information.
  • Exercise sound judgment, discretion, tact and diplomacy while managing highly complex and challenging situations.
  •  Demonstrated ability to work with persons from culturally diverse backgrounds.

Preferred Qualifications:

  • Experience working at a college or university as a case manager or providing direct student support.
  • Experience working as a case manager or as a mental health professional.
  • Master's degree in social work, student affairs, counseling, higher education or related field.
  • Licensed in the State of Minnesota as a Licensed Professional Clinical Counselor (LPCC), a Licensed Psychologist (L.P.), a Licensed Independent Clinical Social worker (LICSW), or Licensed Marriage and Family Therapist (LMFT).
